The Medical Marriage - how to create and maintain a passionate relationship when you are a practicing physician

This week I lead a workshop for medical couples and a very interesting thing happened.

Two long time couples got into arguments in the training

After some detailed questions it became clear that literally the only thing holding their relationships together now is the money. Each physician spouse was literally begging to be a priority in their doctor's life in front of the whole room.

All they wanted was some of their time and attention. In each case the physician's time, energy and awareness was almost completely occupied by their career - and had been so for decades.

The spouse wasn't even playing second fiddle here ... they were a member of the audience at best. Not good.

Quadruple Aim - is it the antidote to Physician Burnout?

[related: Physician Leadership Dirty Little Secrets]

Physician Burnout is the Rodney Dangerfield of issues confronting our rapidly changing healthcare environment

It get's no respect ... at least until now.

In a dramatic mainstream journal article published in the latest issue of Annals of Family Medicine, the authors have proposed using Physician Burnout to redefine the Institute for Healthcare Improvement's Triple Aim for healthcare.

Here is the Triple Aim:

  • Enhancing the patient experience
  • Improving population health
  • Reducing costs

I hope you can see the blind spot in this three part formula

Let's listen to the authors state what should have been blatantly obvoius from the start ...

They note:

Physician Burnout Lessons from Albert Einstein

Physician Burnout Lessons from Albert Einstein

Time and time again I end up quoting Einstein to my physician burnout coaching clients.

The two quotes in this short video are so powerful when it comes to recognizing and reversing physician burnout. Learn them by heart and live them in your practice and your life. You will be way ahead of the game when you do.

I am not sure what the link is between physics and work-life balance. Perhaps there are basic and immutable laws that apply in both arenas. All I know is when I am working with client on physician burnout issues ... all you have to do is memorize these two laws and then live them and you will quickly reverse the downward spiral.

Physician Burnout Vulnerability and Group Culture

Physician Burnout, Vulnerability and Your Group's Culture

I am often asked if I am familiar with the work of Brene Brown and especially her tremendously popular TED videos "The power of vulnerability" and "Listening to shame".

These are fabulous introductions to the topic of vulnerability that can help all of us who see patients on two distinct levels.

Patient Vulnerability:

I highly recommend these videos to all physicians because vulnerability and shame are woven through any authentic healing encounter. It is important to understand and navigate the patient's shame and vulnerability whenever you enter the exam room.

Physician Vulnerability:

Brene Brown's work is also immediately relevant to physician burnout. Vulnerability and shame are experience all of us feel personally when our practice becomes too much. This is especially true for male physicians because of our habit of not reaching out for help.

I love Brene Brown's work but it is incomplete.

Physician Burnout - Why Working Harder Never Works

Physician Burnout - Why working harder never works

In my coaching practice, working with hundreds of over stressed physicians, every single one is a supremely hard worker.

Our residency training values hard work above all else ... to the point of nearly working each of us to death at some point. If there is anything a physician knows how to do ... it is work hard.

It is our default reaction to any challenge. We put our nose to the grindstone and try to overwhelm it by the sheer volume of our effort.

Unfortunately when you come to the dead end of burnout, working harder never works.

Here is why ...

If you work harder doing more of what you are already doing - you will just get more of what you've already got

Code Lavender and Preventing PTSD in Healthcare

Posted by Dike Drummond MD

Code Lavender has been in the news lately as a Burnout Prevention Tool ... that is NOT what it is.

You have heard of Code Blue and Code Red ... well there's a new code in town. Code Lavender.

Code Lavender is a code the hospital staff can call for themselves when they are being overloaded by stress and traumatic events at work.

When Code Lavender is triggered, the staff member gets a Chaplain consult, Reiki treatment, some down time and a lavender ribbon on their arm to tell everyone they are having a rough day. Code Lavender started by Earl Bakken at North Hawaii Community Hospital in Waimea in 2008 and has been reported on most recently in the Cleveland Clinic System.

This week I was interviewed along with Amy Greene, the Cleveland Clinic Director of Spiritual Care, on HuffPostLive on a segment titled:

Code Lavender: Healing One Doctor at a Time. You can watch the interview below.

In the remainder of this blog post (below the video frame) let me show you why Code Lavender is NOT a  burnout prevention tool and the function it actually performs.

Physician Group Culture Trumps Burnout, Maintains Physician Autonomy

Posted by Dike Drummond MD

Your Group's Culture is Key to Maintaining Physician Autonomy and Preventing Physician Burnout

Culture ... What Culture?

The basic challenge is this ...

... most physician groups/departments have never created a Conscious Culture that defines who you are and how you support each other.

This is one of the Key Blind Spots of our current Physician Leadership Vacuum.

A consciously created, supportive Physician Culture is hugely important for all doctors now and in the rapidly changing healthcare environment that is staring us all in the face. Yet the entire concept of a cohesive Physician Culture is completely absent in 85% of the healthcare organizations I interact with across the country.

What about your group/organization?
Does the Culture of your group/department support you in building a healthy practice or actually get in your way?
